Behind every working ventilator is a labour market that is tightening fast. The U.S. Bureau of Labor Statistics counted about 71,800 medical equipment repairers in 2025 and projects the field to grow 13 percent through 2035 — more than four times the 3 percent average across all occupations — with roughly 8,200 openings a year, many of them created by retirements rather than new positions. Translation for a Louisiana clinical engineering manager: the bench you are trying to hire is shrinking while your device inventory grows. Calibration

Biomedical Calibration

Calibration verifies that a device's measurements and outputs match a known reference within stated tolerances — an infusion pump delivering the volume it displays, a monitor reading pressures accurately, an electrosurgical unit producing the power it is set to. When a device drifts out of specification, calibration brings it back or flags it for repair.

Electrical safety inspection is a companion task: measuring leakage current and verifying grounding so that a device does not present a shock hazard to patient or staff. AAMI publishes widely used guidance on electrical safety and equipment management, and this testing is generally performed on a defined schedule with results documented for the equipment record.

Credentials are the shorthand surveyors read first. AAMI's Certified Biomedical Equipment Technician (CBET) exam is open to candidates with an associate degree or higher in electronics, technology or science, a completed U.S. military BMET program plus two years of full-time experience, or four years of full-time BMET experience; the exam runs 165 multiple-choice items, and the credential must be renewed every three years with 30 continuing-education units. It is not a legal requirement to touch a defibrillator — it is simply the fastest way for a facility to show that competency was verified rather than assumed.

Calibration also has a calendar problem that catches busy departments off guard. Test equipment itself drifts, so a shop's electrical-safety analysers, pressure meters and flow analysers need their own traceable calibration schedule; an out-of-tolerance tester quietly certifies out-of-tolerance devices. We keep our reference instruments under a documented calibration cycle and record the standard used on every report, so when a surveyor asks how you know the reading was right, the answer is a document rather than a shrug.

Imaging

Medical Imaging Service

Imaging systems — radiographic and fluoroscopic X-ray, ultrasound, CT, and mammography — combine complex hardware with radiation-producing components, so their service carries both uptime and safety stakes. Downtime in an imaging suite directly delays diagnosis, which is why preventive maintenance and rapid repair matter clinically.

Imaging equipment that produces radiation is also regulated. The FDA regulates radiation-emitting electronic products and mammography under the Mammography Quality Standards Act, and states administer radiation-control programs that may require periodic performance testing. Qualified service, often working alongside a medical physicist for dose and image-quality checks, helps facilities meet those obligations. Specific certification and physics-testing requirements vary by modality and jurisdiction.

Mammography deserves its own paragraph because the rules moved recently. The FDA's amended MQSA regulations — published as a final rule in March 2023 — became enforceable on 10 September 2024, and they now require every mammography report to classify breast density and every patient lay summary to carry a density-notification statement. The equipment side of that rule has not changed shape: accredited facilities still run daily and periodic quality-control tests and an annual survey by a qualified medical physicist, and a unit that fails its phantom or dose checks does not image patients until it is corrected.

Imaging is also where CMS draws a hard line on maintenance flexibility. A hospital or critical access hospital may move maintenance intervals off the manufacturer's recommendation under a risk-based AEM program — but not for imaging or radiologic equipment, and not for medical laser devices, which must stay on manufacturer-recommended activities and frequencies. If your CT, R/F room or portable is on an in-house 'we do it when it looks off' schedule, that is a finding waiting to happen, and it is one of the first things we straighten out.

Isolated power system and line isolation monitor testing

NFPA 99

Isolated Power Testing

Isolated power systems (IPS) are used in certain wet procedure locations — most commonly operating rooms — to reduce the risk of shock and to keep power available even in the event of a single ground fault. A line isolation monitor (LIM) continuously watches the system and alarms if leakage current rises toward a hazardous threshold.

NFPA 99, the Health Care Facilities Code, addresses the design and testing of these systems, and facilities generally verify LIM function and system performance on a periodic basis. Because the alarm is the safety mechanism, routine testing that confirms the monitor actually trips at the correct threshold is central to the work. Specific test intervals and thresholds should be confirmed against the current edition of NFPA 99 and the manufacturer's instructions.

The current edition of the Health Care Facilities Code is NFPA 99 (2024). Its line-isolation-monitor provisions are specific enough to plan a route around: the LIM circuit is tested after installation and before being placed in service by successively grounding each line of the energised system through a resistor of 200 × V ohms, where V is the measured line voltage; thereafter the test switch is actuated at intervals of not more than one month, extended to not more than 12 months only where the monitor has automated self-test and self-calibration capability. Actuating the test must produce both visual and audible alarm.

Two details trip up facilities that otherwise run a tidy program. First, a LIM test is also required after any repair or renovation to the isolated system — the contractor who reworked an OR panel last quarter almost certainly did not perform it. Second, a monthly button-push logged by whoever happened to be in the corridor is not the same as a documented functional verification against a known fault load. We do the fault-load test, log the trip point, and hand you a record that stands up in a survey rather than a checkmark that does not.

Laboratory

Clinical laboratory instruments — chemistry and hematology analyzers, centrifuges, incubators, and refrigeration — produce the results clinicians rely on to diagnose and treat, so their accuracy is directly tied to patient care. Preventive maintenance, calibration, and temperature verification are routine parts of keeping a lab dependable.

Laboratory testing is regulated under CLIA, administered by CMS, and many labs also pursue accreditation from bodies such as the College of American Pathologists (CAP), whose checklists address instrument maintenance and function verification. Well-documented service records support both CLIA compliance and CAP inspection. The exact requirements depend on the test complexity and the accrediting organization involved.

CLIA sorts tests by complexity, and the obligations scale with it: waived testing carries the lightest burden, while moderate- and high-complexity testing brings quality-control, calibration-verification and personnel requirements that inspectors examine closely. CAP checklists go further still, asking laboratories to show function verification and documented maintenance for each instrument on the bench. None of that is satisfied by a technician's memory. It is satisfied by a dated record naming the instrument, the procedure, the reference standard and the result — which is precisely the artefact our service produces as a by-product of doing the work.

Cold storage is the quiet risk in most laboratories. Reagent refrigerators, specimen freezers and incubators fail slowly and silently, and a drift of a few degrees can invalidate results long before anyone notices a warm door gasket. Continuous monitoring with calibrated probes, documented alarm testing, and a mapped temperature profile for each unit turn an invisible risk into a managed one — and turn a bad night into a two-hour recovery instead of a week of repeat testing and awkward conversations with clinicians.

Dialysis

Hemodialysis depends on both the dialysis machines and the water treatment system that feeds them. Because a patient's blood is exposed to large volumes of dialysate derived from that water, water quality is a safety-critical concern, and the supporting equipment — pretreatment, reverse osmosis, and distribution — must be maintained alongside the machines themselves.

Dialysis facilities operate under CMS conditions for coverage, and the industry widely references AAMI water-quality standards for hemodialysis. Routine maintenance, disinfection, and water testing are part of keeping a unit compliant and safe. Specific monitoring and testing requirements should be verified against current CMS regulations and the applicable AAMI standards.

Water is the largest single input in hemodialysis, and it is the one nobody sees. A patient on a standard thrice-weekly schedule is exposed to a volume of dialysate that dwarfs anything taken orally, which is why the AAMI water-quality standards for hemodialysis — addressing chemical contaminants, microbial limits and the design of the treatment and distribution system — are referenced so widely across the industry, and why ESRD facilities operate under CMS conditions for coverage that expect that water system to be maintained, disinfected and monitored on a defined schedule.

Practically, that means pretreatment media that is actually exhausted-and-replaced rather than assumed good, RO membranes trended instead of guessed at, distribution loops disinfected on a documented cycle, and machines whose conductivity and temperature alarms are verified rather than trusted. Louisiana's heat and humidity are not kind to feed-water systems, and a unit that quietly drifts is far more expensive than a unit that is measured. We would rather find the drift on a Tuesday PM than have you find it on a Saturday treatment.
